045 Chronic Fatigue Syndrome (CFS) and Post-Acute Sequelae of SARS-CoV-2 Covid19 infection (PASC)

In this video Doctor Furlan explains the difference between tiredness and chronic fatigue syndrome (CFS). The technical name of CFS is myalgic encephalomyelitis (ME). Fatigue is synonym of tiredness, weariness, exhaustion and lethargy. It seems that Post-Acute Sequelae of SARS-CoV-2 Covid19 infection (PASC) is very similar to ME/CFS, so we will see many more people with this condition in the future. The diagnosis of ME/CFS is according to the symptoms of severe profound fatigue that does not get better with rest. The fatigue is caused by post-exertional malaise (PEM). There is a test called two-day cardiopulmonary exercise test (CPET) that is used to measure anaerobic threshold. The main symptoms of ME/CFS include: - Fatigue: physical and mental - They will wake up unrefreshed. - There are abnormalities in the heart rate - Intolerance to standing position, so the person always prefers to lie down in horizontal position (orthostatic intolerance) - Sensitivity to sound, noise, smells and chemicals - There is muscle and joint pain. And here is where there is overlap with fibromyalgia. It is important to exclude any other common causes of tiredness: - Anemia - Heart, kidney or liver failure - Low thyroid function, known as hypothyroidism - Sleep diseases such as sleep apnea - Side effects of medications like blood pressure medications, antidepressants, and opioids - Some psychiatric conditions like depression or anxiety For patients with ME/CFS Dr. Furlan will talk about the 6M. - Movement including pacing your physical activity, gentle exercises, gradually increasing the intensity and duration. Yoga, Tai Chi, going for walks, stretching at home, all of these are excellent. - Mind-body includes time for yourself, relaxation, meditation, praying, singing, meeting people that you like. It also includes a more structured psychological therapy such as hypnosis, or cognitive-behavioural therapy CBT. - Modalities include application of heat or cold, a shower, or a bath. - Manual therapy includes self-massage, going to a massage sessions or spinal manipulations - There is no medication for ME/CFS - The sixth M is for Meals. It is very important to remember that you are what you eat. Changing your eating habits might seem hard in the beginning, but you don’t need to start making drastic changes. Just start by adding some healthy fats to your diet, caffeine in the morning, smoothies, more fruits and vegetables during the day. Remember, your diet starts with your grocery shopping.
